Establish Power of Attorney (POA)
Complete the Power of Attorney form to give legal authority to one or more people you name as your agent to make decisions on your behalf regarding your SERS benefit. Then mail your form to the address shown, or deliver it to one of our field offices across the state.
If you already have a general POA, you may submit that document to SERS.
SERS staff will review your form or general POA and notify you, and in some cases your newly designated agent, when it has been approved and added to your member record.
You may authorize your agent to perform, on your behalf, any SERS transaction that you could request yourself. You can also limit the power your agent has over specific transactions. For example, if you do not want your agent to be able to select or change your retirement option, you can place a check mark in the appropriate space in section A of the form. In addition, you may revoke the Power of Attorney at any time.
